Electronic Exchange of Data and “Closing The Loop” (2022). Resource Type: Archived Webinar. Description: Building the electronic exchange of data directly into the EHR is needed to reduce inefficiencies, document patient connection with available legal services, and track patient legal outcomes. This Case Study provides a concrete example of how a medical-legal partnership (MLP) in Iowa set up electronic referrals in the EHR as well as the ability to “Close the Loop” via electronic updates from the legal partner. Description: The toll of homelessness has been shown to have long-term consequences on health and quality of life. What is not as well documented is the impact of homelessness on mortality and the incidence of specific causes of death. Homeless mortality is often underestimated due to lack of consistent housing status reporting on death certificates and incomplete data from various siloed record systems. Understanding the extent and cause of mortality among people experiencing homelessness is necessary to know how to prevent future mortality. This webinar will highlight the progress of four counties in the Bay Area, California, Alameda, Marin, San Francisco, and Santa Clara, who have worked to identify and investigate homeless mortality. As such, it is becoming crucially important to understand the different strategies and partnerships that have emerged from this crisis. Health centers have an important and essential role in addressing the health-harming social and legal needs of their patients. The National Center for Medical-Legal Partnership (NCMLP) is conducting an environmental scan of health centers with the main goal of establishing, expanding, and sustaining the accessibility and availability of medical-legal partnerships to meet the evolving social needs of patient populations.
