Resource Summary: After understanding the roles and responsibilities of health center staff in care management, this session will focus on the tools required to document longitudinal care management activities. The session speakers will review two (2) common electronic health record systems and provide details on how to document care plans and care management activities that meet Patient-Centered Medical Home (PCMH) standards.
Resource Details: Participants will learn to identify how PCMH integrates with a diabetes care management program; restate the care management process for individuals with diabetes, and identify the National Committee for Quality Assurance Electronic Medical Record requirements for diabetes care management programs.
