Resource Details: The purpose of this publication is to share lessons learned from health centers and other community-based partners that are implementing new strategies for preventing diabetes in their communities. These lessons emerged from Learning Collaboratives hosted by the Corporation for Supportive Housing (CSH) from 2021-2022, which were comprised of health, housing, and homeless services providers. The results will be useful for community-based providers as they initiate new prevention strategies or improve their existing efforts.
