Introduction

This resource brief provides a selection of current, high-quality resources about diabetes and its public health impact. Topics range from the need for screening and treatment to special focuses on how to conduct successful outreach to special and vulnerable populations.

In recognition of November as National Diabetes Awareness month and HRSA’s Bureau of Primary Health Care Diabetes Quality Improvement Initiative, which addresses diabetes prevention and management, as well as pediatric overweight and obesity prevention, the following highlighted resources have been identified to support health centers as they continue to innovate and deliver essential care to patients across the country

About the Issue

Diabetes mellitus (DM) occurs when the body doesn’t produce enough or respond appropriately to insulin, which is needed to absorb and use glucose (sugar) to fuel the body. “Without a properly functioning insulin signaling system, blood glucose levels become elevated and other metabolic abnormalities occur, leading to the development of serious, disabling complications.”1 Healthy People 2030 has established 15 objectives to reduce the prevalence of the disease, increase diagnosis, and improve care. Evidence-based professional information/recommendations and consumer information have been collected nationally along with aggregated national data.

Diabetes Clinical Management and Periodontal Care for Diabetic Patients Individuals Experiencing Homelessness

Abstract: This session will focuses on providing up-to-date standards of the American Diabetes Association guidelines for screening diabetic patients with periodontal disease, clinical recommendations for treatment and management of patients diagnosed with diabetes, the role of oral health and dental conditions for effective screening, and health center experiences in supporting patients in diabetes management. Successful strategies for how health centers have adapted their treatment and management of diabetes for individuals experiencing homelessness are also shared. Impact of the Workplace on Primary Care Management of the Diabetic Patient

Abstract: Management of diabetes can be affected by many factors including the type of job of the diabetic patient. More importantly, poor diabetes management can compromise a workers’ ability to perform their job duties. The standard of care to regulate blood glucose such as standardized medication, activity, and food regimens may not be possible for many workers, particularly those in low-wage jobs. There may be no place to store medication or food, breaks may be limited or not regularly be offered and following a food regimen while working may not be feasible. This webinar reviews how work can impact diabetes management and offers strategies and tools to assist diabetic patients in managing their disease at work. Results from a recent study with primary care providers will be shared to highlight new recommendations for clinical decision support. Abstract: Understanding and Addressing Diabetes in your Community: A Quick Guide for CHWs is a resource for Community Health Workers that provides information about diabetes, and provides tools for Community Health Workers can use to guide patients with diabetes towards a healthier future. Included with the guide are guidelines on how to manage diabetes, as well as an A1c tracker so patients can track their progress towards their A1c goals. Endo ECHO

Abstract: The Endocrinology TeleECHO Clinic (Endo ECHO) trains and supports primary care providers in rural and underserved communities to improve their knowledge of and skill in the care and management of patients with endocrinologic needs. Issues like complex diabetes, thyroid disease, adrenal and pituitary disorders, hypogonadism, Polycystic Ovary Syndrome (PCOS), hyperlipidemia, metabolic bone disorders and transgender health are discussed weekly in a community of practice with a team of experts.
