Resource Summary: Chronic diseases impact one of every two adults and account for 86% of our nation\'s healthcare spending. These conditions also have a disproportionate impact on traditionally marginalized communities including racial and ethnic minorities, LGTBQIA+ community members, recipients of HUD funding and Residents of Public Housing. Fortunately, many chronic diseases such as cardiovascular disease and type-two diabetes mellitus are preventable, and patients can now live long and healthy lives when provided with the appropriate supports.
Resource Details: In this webinar, Dr. Kevin Lombardi MD, MPH and the NCHPH Research and Clinical Quality teams presented a comprehensive overview of the latest clinical, programmatic and social guidance for chronic disease prevention, treatment and support. Covered chronic diseases included cardiovascular disease, type-two diabetes mellitus, COPD and conditions of chronic pain with consideration also given to HIV and Rheumatological conditions.
