Resource Summary: This webinar provides an overview of postpartum care coordination and education as an effective intervention for improving maternal and infant health. The webinar will discuss evidence-based home visiting models, connection to and establishment of a primary care medical home, and health insurance coverage. Participants learn about the importance of postpartum education for infant safe sleep, family planning, and chronic and behavioral health issues.

Resource Details: Participants will be able to describe how community connections and education supports positive health outcomes after deliver; articulate how to empower and educate patients on how to recognize and report postpartum concerns and complications; identify the social drivers of health that impact postpartum recovery, and collaborate with other community-based organizations to provide the continuity of care that improves maternal health outcomes.
