Resource Summary: This webinar shares how to build a collaborative model where nurse care managers, payer partners, community health workers/providers/patient navigators, or pharmacy staff connect to manage patients with diabetes. The information from this webinar will help health centers build relationships with their patients to have a positive care management and care coordination experience since care management is used to improve patient care, quality, and health outcomes. The Patient-Centered Medical Model sets expectations that healthcare organizations use care management to help patients develop and meet treatment plan goals.
