Resource Summary: This webinar will discuss how to connect with local hospitals, obstetrics practices, health departments, and diagnostic imaging groups to improve access to maternal health services in rural and urban communities. The purpose of this training is to introduce ways in which clinicians who care for pregnant people can more easily connect them with local resources to improve outcomes.
Resource Details: Participants will be able to describe the importance of community connections for maternal health patients; explain which social drivers impact pregnancy outcomes; identify which platforms or services would benefit your patients; determine and articulate which data you will track to monitor outcomes; and identify processes, procedures, or tools needed within one's organization for implementation of a community connection platform for both urban and rural patients.
