Resource Summary: Food Prescription (Rx) programs offer innovative solutions to addressing social drivers of health (SDOH), including food insecurity, that impact people with chronic conditions through a model that builds partnerships between the local food system, the healthcare system, and the patient population. This presentation will familiarize attendees with Food Rx programs, the NCFH Food Rx Replication Guide, and demonstrate how to use this resource to create their very own personalized Food Rx programs to improve health outcomes at the fundamental level in partnership with local organizations.
