Resource Summary: Health centers need to understand the resources and how to access to support affordable housing for their clients. Rapid Rehousing is an effective housing intervention to quickly stabilize individuals and families to address their health and service needs.

Resource Details: Health centers are increasingly addressing the social determinants of health for their patient population through partnerships and linkages to local housing and service resources. The goal of rapid re-housing is to help individuals or families (with or without disabilities) who are experiencing homelessness, to move as quickly as possible into permanent housing and achieve stability. Health centers can play an important role in connecting these households to services.
