Resource Summary: This publication is part of a series of case studies that highlight innovative models for providing enabling services to help reduce barriers to care and address social determinants of health. This case study features Wellspace Health's (WSH) "Health, Access, Referrals, and Transitions" (HART) program. WSH's HART program utilizes case management services to connect patients to care and to make care transitions to critical clinical and behavioral health services. The HART program provides intensive case management and addresses substance use disorders, suicide prevention, provides street outreach, and respite and recuperative services.
