Resource Summary: This pamphlet was developed as a companion for health centers asking their patients’ sexual orientation and gender identity. This handout walks patients through why they are being asked these questions and what all of the answer options mean. Patients will be guided on how to choose the best answer, as well as what to do if they do not want to answer these questions. There is also information for patients about why this information is important to the health center, and what will be done with the answers to these questions.
