Resource Summary: HOP’s Outreach Reference Manual chapters provide direction for how to most effectively use outreach to increase access to and utilization of comprehensive primary health care services across many underserved communities. This chapter guides readers through participatory planning methodologies and the development of three key frameworks: 1) a logic model that articulates what the program’s impact will be, 2) a work plan that outlines how the program will achieve that impact, and 3) an evaluation plan that outlines how the program will evaluate if it was successful.
